Does HMO Cover Therapy in Nigeria 2026? NHIS BHCPF + Private HMO Guide

“Therapy sounds good, but can my HMO pay for it?”

If that’s the question stopping you, you’re not alone. After “how much is therapy”, this is the second most-asked question we get at MindMed.

Mental health coverage in Nigeria 2026 is changing, but it’s still patchy.

5 years ago, most HMOs said, “mental health is not covered”. Today, NHIS now includes mental health under BHCPF, and some private HMOs cover therapy. The problem? Most people don’t know how to check what’s actually covered, or what to ask HR.

This guide shows you exactly how HMO therapy works in Nigeria right now. NHIS BHCPF reality + private HMO coverage for Hygeia, AXA Mansard, and Reliance HMO. Plus 3 workarounds if your HMO says no.

HMO and Mental Health Before 2022 vs 2026

Before 2022: Most Nigerian HMOs treated therapy like a “luxury”. Not covered. If you wanted therapy, you paid out of pocket.

2026 reality:  2 big shifts happened:

1. NHIS Act 2022 + BHCPF: Mental health is now part of the Basic Health Care Provision Fund. On paper = covered.

2. Corporate pressure: Companies now add “mental wellness” to HMO plans to retain staff. Burnout costs companies more than therapy.

But here’s the catch: “Covered on paper” ≠ “easy to access”. Coverage depends on your HMO, your company plan, and how you ask.

NHIS + BHCPF: What’s Actually Covered in 2026?

Under NHIS Basic Health Care Provision Fund, mental health is now listed as a benefit. 

What’s covered on paper:

- Basic psychotropic medicines on NHIS drug list

- Consultation with doctor/psychiatrist at primary health centers trained under mhGAP

- Referral to specialist if needed

Reality in 2026:

  1. Therapy vs drugs: NHIS benefit package covers basic psychotropic medicines but excludes non-pharmacological interventions. Translation: drugs yes, 6 sessions of CBT/talk therapy = hard to find via NHIS currently.

  2. Access is slow:  Not every primary health center has a trained mental health worker yet. BHCPF rollout and mhGAP training is ongoing. Lagos State MeHPriC program trained 852 PHC workers, but scale-up is still limited.

  3. Privacy concerns. Many Nigerians won’t go to local PHC for mental health due to stigma.

 

Bottom line: NHIS made mental health legal + fundable. But for most working Nigerians with private HMO, private HMO coverage matters more right now for actual therapy sessions.

Private HMO Reality: Hygeia, AXA Mansard, Reliance HMO

Good news: Private HMOs are moving faster than NHIS.

What some HMOs cover in 2026:

- AXA Mansard: Corporate plans have flexibility to add Psychiatric treatment. Some plans include Psychiatry Cover up to 8 weeks. AXA UK model shows 6-12 counseling sessions are standard when mental health is covered. Nigeria plans vary by employer.

- Hygeia HMO: Higher plans include Nutritionist Cover, Telemedicine. Mental health counseling depends on the corporate package. Unlimited outpatient limit ₦500,000 on premium plans.

- Reliance HMO: Known for digital access + fast authorizations

-Reliance Wellness add-on covers virtual therapy for some corporate clients. Sessions 4-8 depending on plan. 

What research shows: Private insurance provides minimal mental health coverage, typically limited to psychiatrist consultations without psychologist services or intensive therapy. Translation: HMO will cover “see psychiatrist” more easily than “see psychologist for 10 sessions”. 

3 things to check on YOUR HMO card:

  1. Is “mental health”, “psychiatry”, “counseling”, or “wellness” listed?  Check the benefits page or ask customer care.

  2. How many sessions per year? 6-20 sessions per year is the common range globally. Nigeria's plans are similar.

  3. In-person or online? Many HMOs now approve online therapy. Easier than finding an in-network clinic.

 

Script for HR/Your Company:

“Hi HR, does our HMO plan include mental health or counseling benefits? I saw NHIS now covers it under BHCPF. Can you confirm sessions per year + if online therapy is allowed?” 

Companies say yes more often when employees ask. Silence = no coverage.

When Your HMO Won’t Pay: 3 Workarounds That Work

If customer care says “not covered”, don’t give up. Try these:

Workaround 1: Employer Wellness Budget

Many Nigerian companies now have a separate “wellness budget” outside HMO. 50k-200k per staff yearly. Ask HR: “Can therapy come from the wellness budget instead of HMO?” Frame it as productivity.

Workaround 2: FSA-style Claims + Reimbursement

Some HMOs won’t pay therapist directly, but will reimburse after. Pay cash → get receipt + doctor’s note → submit claim. Ask HMO: “Do you reimburse mental health consultations?” *Co-payments + partial reimbursement* is how most insurance covers therapy globally. 

Workaround 3: Start Small + Affordable

  2. Call customer care: Ask “Is therapy/psychological counseling covered? How many sessions? Online allowed? Preauthorization needed?”

  3. Ask HR: Forward them this guide. Companies can upgrade plans next renewal if staff ask.

 

Pro tip: Don’t say “I’m depressed”. Say “I want to use my mental wellness benefit for stress management”. Less stigma, same service.

Coverage is Growing, But You Must Ask

Nigeria 2026 is not 2020. Mental health is now in NHIS law + BHCPF and corporate HMO plans. But HMOs won’t advertise it. You have to ask.
